Questions about treatment
Why does my Elvanse wear off early?
Elvanse does not provide the same length of benefit for everyone. An early afternoon dip can have several explanations. Record the pattern and discuss it with your prescriber before changing the dose or adding another medicine.

- Elvanse does not provide the same length of benefit for everyone.
- An early afternoon dip can have several explanations.
- Wearing off, rebound and tolerance are different things.
- Record the pattern and discuss it with your prescriber before changing anything.
Does early wearing off mean Elvanse is failing?
Not necessarily. A medicine can help during part of the day but leave an important gap later.
Also consider what “wearing off” means for you. Are familiar ADHD difficulties returning? Are you tired or hungry? Or are you feeling unusually anxious, flat or physically unwell?
These experiences can overlap. Timing is useful evidence, but it does not diagnose the cause.
Is it wearing off, rebound, not enough benefit, or tolerance?
These are different things, and they need different responses. Timing is a clue, not a diagnosis.
| What you notice | What it may be | What to tell your prescriber |
|---|---|---|
| Your usual ADHD difficulties come back in the afternoon or evening | The medicine wearing off | When benefit starts and when it fades |
| A short spell of feeling irritable, flat, anxious or very tired as it wears off, sometimes called a crash | Rebound | How long it lasts and whether it happens most days |
| Little benefit at any time of day | Not enough benefit from this dose or medicine | What has improved, if anything |
| A medicine that helped for months now helps less at the same dose | A change in response; some people call this tolerance | When the change started and anything else that changed |
None of these means the dose must go up. Your prescriber will look at the whole pattern.
What should I check?
| Check | What to record |
|---|---|
| Dose | Prescribed amount and time actually taken |
| Benefit | When useful changes begin and when difficulties return |
| Symptoms | What changes, rather than only “it stopped working” |
| Sleep and meals | Sleep duration, missed meals and appetite |
| Other products | Medicines, supplements, caffeine and recent changes |
| Daily demands | Whether the difficult period involves more demanding tasks |
Use a few typical days. Do not delay seeking help to complete a diary if symptoms are severe.
Should Elvanse last 14 hours?
Fourteen hours is a finding from adult clinical studies, not a promise for each person. Study duration also does not mean the medicine’s effect feels equally strong throughout that time.
The practical question is whether treatment covers the activities that matter to you without causing unacceptable side effects.
What can my prescriber change?
Your prescriber can review the dose, timing, side effects, other medicines and whether a different treatment would suit you better.
An increase is not automatically the answer. Do not split capsule contents into separate doses, take an extra capsule or add a short-acting stimulant without a prescription.
When should I seek help sooner?
Contact your prescriber promptly if the dip repeatedly affects work, relationships or safe driving, or includes persistent low mood.
For severe chest pain, severe breathing difficulty, a seizure or signs of a stroke, call 999. Use the Elvanse guide’s “When to get help” section for other warning signs.
Common questions
Does wearing off early prove I have developed tolerance?
No. A change in benefit does not establish the cause. Your prescriber should review the whole pattern rather than assuming either that tolerance is present or that it is impossible.
Will a high-protein breakfast make it last longer?
There is no basis for promising that a particular breakfast will extend Elvanse’s action. Regular meals can help you manage appetite and energy. Elvanse can be taken with or without food.
Should I take it later to cover the evening?
Discuss this first. Moving it later may leave your morning uncovered and interfere with sleep.
