Questions about treatment
Why does Concerta XL seem to stop working after lunch?
Concerta XL is designed to provide benefit for up to 12 hours, but your experience may differ. Afternoon difficulties do not automatically mean you need a higher dose. Check the exact product and record what changes before discussing it with your prescriber.

- Wearing off, rebound and tolerance are different things.
- Check the exact product and record what changes before you discuss it with your prescriber.
Am I definitely taking Concerta XL?
Check the name and formulation on your pack. Other methylphenidate products may release medicine differently, even when the dose number looks similar.
If your pharmacy supplied a different product, check that the change was agreed. Do not assume that all “XL” medicines have the same daily pattern. See switching methylphenidate brands safely.

What could explain the afternoon change?
| Pattern | Useful information for your review |
|---|---|
| Little benefit at any time | Whether the dose or medicine has provided a meaningful response |
| Benefit earlier, familiar difficulties later | Timing and duration of useful coverage |
| Irritability, anxiety or feeling flat | Whether these occur while the medicine is active or as benefit fades |
| Tiredness after poor sleep or missed food | Sleep, appetite and meal pattern |
| A change after a new pack | Exact product, strength and date of the change |
This table helps organise a discussion. It cannot identify the cause on its own.
Is it wearing off, rebound, not enough benefit, or tolerance?
These are different things, and they need different responses. Timing is a clue, not a diagnosis.
| What you notice | What it may be | What to tell your prescriber |
|---|---|---|
| Your usual ADHD difficulties come back after lunch | The medicine wearing off | When benefit starts and when it fades |
| A short spell of feeling irritable, flat, anxious or very tired as it wears off, sometimes called a crash | Rebound | How long it lasts and whether it happens most days |
| Little benefit at any time of day | Not enough benefit from this dose or medicine | What has improved, if anything |
| A medicine that helped for months now helps less at the same dose | A change in response; some people call this tolerance | When the change started and anything else that changed |
None of these means the dose must go up. Your prescriber will look at the whole pattern.
Does lunch stop Concerta XL working?
Concerta XL can be taken with or without food. Do not assume that lunch switches the medicine off.
Eating regularly is still useful, especially if treatment reduces appetite. Report a consistent pattern rather than changing meals or medication to test a theory yourself.
What can my prescriber consider?
They can review dose, response, tolerability and whether another formulation would fit your day.
NICE includes a morning modified-release methylphenidate preparation with a prescribed immediate-release dose later as one possible way to extend coverage. This needs an individual prescribing plan. Do not add a dose yourself.

When should I seek help sooner?
Tell your prescriber promptly about troublesome mood changes, persistent poor appetite or difficulties affecting safety.
Severe chest pain, severe breathing difficulty, a seizure or stroke signs need emergency help. Other warning signs are listed in the Concerta XL guide.
Common questions
Does seeing an empty tablet shell mean the dose failed?
An empty shell can pass in your stools because it does not dissolve. That can be normal. It does not tell you whether the treatment gave enough benefit or prove how much medicine was absorbed.
Can I crush the tablet to make it work faster?
No. Concerta XL must be swallowed whole. Crushing, chewing or dividing it disrupts the release system.
Does early wearing off always mean the dose is too low?
No. Your prescriber needs to consider benefit, side effects, timing and the exact product before deciding what to change.
